Objectius
Homelessness is an ever-growing European challenge now affecting more social groups than ever. The EU recognises combating homelessness as a key policy priority, highlighting the need to provide preventive, agile and targeted responses. Homelessness response in Europe is inherently decentralised and atomised, hindering the effectiveness of homelessness response and theidentification of suitable, customised and cost-effective interventions for the diverse needs of the homeless population. Effectively addressing homelessness requires an integrated approach, encompassing housing and support services, but also prevention and the sharing of evidence-based practices and guidelines among all European stakeholders. COMHOM will contribute to the digitalisation of the homelessness sector by bringing technology closer to these actors and making it accessible to them, building their capacity and facilitating technology integration. To this end, the COMHOM concept includes a standardised M&E framework, digital tools (platform, database and AI models) and capacity-building programmes, with the ultimate ambition of achieving a more integrated approach to homelessness across Europe. COMHOM relies on data collection, sharing, complex data analysis and AI-based models (a rules engine and an AI-enhanced algorithm) to support decision-making and cover the continuum from prevention to intervention, including learning from the ongoing M&E it conducts. The COMHOM concept will be co-created in a participatory process that will ensure ownership and acceptance in the sector, which will be tested in two social experimentation intended in this project – achieved through sub-granting 25 European homelessness response sub-grantees. The project will support the EU policy priorities and the first and second work strands set forth by the European Platform on Combating Homelessness.

Objectius
Este proyecto tiene como objetivo principal desarrollar e implementar una intervención online para prevenir losproblemas mentales comunes y promover la salud mental y el bienestar en el lugar de trabajo. Para ello, se implementarán una serie de mejoras en la intervención EMPOWER, desarrollada dentro de un consorcio europeo financiado por el programa H2020, que consiste en una campaña antiestigma y de concienciación sobre la salud mental laboral y una aplicación móvil. Esta intervención ha sido implementada mediante un ensayo aleatorizado en 4 países europeos, incluyendo España, y se encuentra en un nivel de madurez tecnológica de 5-6. Para la versión mejorada, se tendrán en cuenta la información cualitativa y cuantitativa del proyecto EMPOWER, se realizarán consultas con empresarios/as y trabajadores y se realizarán versiones en castellano y catalán. La versión mejorada se pondrá a prueba en empresas de la región de Cataluña mediante un estudio observacional, en el que se obtendrán datos sobre salud mental, presentismo y ausentismo a nivel basal, al cabo de 7 semanas de uso y a los 3 meses. Estos datos se analizarán para determinar la efectividad de la intervención. Además, se realizarán entrevistas cualitativas con los managers y trabajadores para conocer la usabilidad y aceptabilidad de la intervención. Por último, se analizarán datos de uso de la aplicación (engagement). Se espera alcanzar un nivel de madurez tecnológica del 8-9.Desarrollo de un modelo predictivo con inteligencia artificial para la detección temprana de insuficiencia respiratoria en pacientes pediátricos. Objectius
The proposal addresses the urgent global epidemic of obesity, impacting billions of individuals worldwide. Obesity significantly elevates the risk of chronic conditions such as type 2 diabetes and cardiovascular disease, imposing substantial burdens at both individual and community levels, as well as incurring enormous economic costs for healthcare systems.Current guidelines mainly focus on acute treatment rather than long-term therapy, with important differences based on gender and socioeconomic background. Moreover, traditional community-based health strategies face significant challenges in effectively reaching and supporting vulnerable populations like young individuals, low-income families, and migrants. These challenges significantly contribute to health inequalities and hinder the effectiveness of current programs. This highlights the urgent need for novel multidisciplinary approaches with patient engagement at its core to implement effective prevention strategies. The project aims to develop and implement an innovative and interactive platform (CONNECT-CARE platform) to empower children and adolescents in managing their health and adopting healthier lifestyles. With ground-breaking digital tools at its core, this new platform will go beyond traditional prevention strategies by reshaping the way young individuals’ access and engage with healthcare systems. Through implementing a streamlined and bidirectional user-healthcare interaction as well as introducing an Artificial Intelligence (AI)-assisted personalized obesity management approach, the project will transform how young individuals receive and perceive health messages. To achieve these aims, the project will use data from primary healthcare systems, encompassing clinical, socioeconomic, behavioural, and cultural aspects, to allow for the development of a pan-European Youth Empowerment Hub (YEH), which will harbour all digital tools and resources including the personalized obesity management platform (NutriFit Advisor) and an online educational platform. The YEH will be seamlessly integrated into the healthcare system for a bidirectional interaction between users and health professionals, improving healthcare providers' capacity to deliver services efficiently. The feasibility and effectiveness of the YEH and its digital solutions, will undergo evaluation in two randomized-controlled pilot studies in Spain and Denmark. The implementation of a bidirectional communication platform with AI at its core will serve as a transformative tool, reshaping the relationship between users and healthcare providers. By facilitating seamless interaction, it will enhance healthcare access for users while simultaneously improving service delivery for healthcare providers. Furthermore, the project will establish the AI-assisted NutriFit Advisor as a trusted and reliable tool for young individuals, addressing a critical need for more tailored and effective health interventions. Finally, the emphasis on prevention, supported by better use of tools and data to improve the quality of obesity care, will contribute to more cost-effective and personalised care. In summary, this project will shift the paradigm of healthcare delivery towards more patient-centred and digitally-enabled models transforming health and care in daily life, and ultimately leading to better health outcomes.
SPINE-SYNC: Multi-stakeholder platform for scoliosis education, communication, professional collaboration and health literacy.

Objectius
The main objective of SPINE-Sync is to improve the care journey of paediatric scoliosis patients through a digital platform that promotes organisation across different levels of healthcare, facilitates professional training and communication, and increases health literacy and empowerment of patients and families. Specific objectives (SO):SO1. Establish a transnational, multi-stakeholder learning community on the scoliosis patient journey (early detection, secondary and tertiary prevention and treatment). SPINE-Sync will build a learning community with healthcare professionals, patients and families from different healthcare systems. It will promote collaboration, knowledge networking and continuous and shared learning.
SO2. Co-design, develop and evaluate the SPINE-Sync platform. SPINE-STEP aims to become a hub for healthcare professionals, patients and families dealing with scoliosis.
The platform will include training resources, health literacy strategies to empower and enable patients and families, and communication channels. The digital platform will consist of three primary interfaces:
1) Professional training to enhance knowledge sharing and promote up-to-date scoliosis practices.
2) Health literacy to empower patients and families by providing reliable and understandable information.
3) Communication and collaboration to facilitate interaction between stakeholders. The evaluation of the relevance and feasibility of SPINE-Sync will include qualitative and quantitative approaches using key indicators such as end-user satisfaction and well-being, adherence and reduction of unnecessary referrals.
SO3. Define the roadmap for integrating the platform into health services. Based on the findings of the co-creation process, SPINE-Sync will plan the integration of the digital platform into mainstream healthcare services as a tool and strategy to facilitate continuity and personalisation of care.
